This site has an index to the soldiers included. There were two sets of questions. Mostly the questions center on economic status before war, interaction between slave owners and non-slave owners before the war, whether soldier's family owned slaves, whether slave ownership was helpful in winning elections, as well as the education of the soldier, his war experiences and status of the soldier after the war. Two questions ask about the parents of the soldier. The questions would seem to be an attempt to correct what the state archivist felt to be mistaken views of the South and the war. I am not aware of any other state that sent out questionnaires.
