I would think it is sort of like looking at an undercover cop and realizing that is what he is.
An undercover cop carries himself in an erect manner,
speaks clearly, and carries himself in such a way that he stands apart from the others.
Changing the clothes would not change the man nor an erect posture.
So yes, I understand what is meant by his appearance.
