D.G. Fulghum

I am looking for any information on my gggrandfather D.G. Fulghum who was in the Crescent Louisiana Infantry Co. B which later became the Consolidated Crescent Regiment Louisiana Infantry Co. D. This paragraph is the only thing I have ever seen with his name on it.
Nov. 3, 1863 the Crescent Regiment consolidated with the 16th Battalion Louisiana Infantry AKA Confederate Guards Respomse Battalion and the 11th Battalion Louisiana Infantry. The new organization designated as the Consolidated Crescent Regiment Louisiana Infantry.
From the Book "Records of Louisiana Confederate Soldiers and Louisiana Confederate Commands Vol. II" D.G. Fulghum appears on a Register of C.S.A. General Hospital, Shreveport, La. Admitted March 31, 1864. Transferred April 6, 1864 to Jefferson Texas Hospital. Enlisted Jan. 8, 1863, Bossier Parish, La.
I have tried to find any place I could about the hospital in Shreveport and the one in Texas but have not been able to find anything. How do I find his medical records or anything about him or his units. Any help would be appreciated.
