Go to a trophy shop and have them engrave on metal your wording. A monument service may have a suitable flat stone to place the plate upon. Another idea would be to buy a concrete block large enough or pour one yourself to place the metal plate upon. If you poured the block yourself, you could insert long bolts in the concrete to hold it in place. Then you could place the flat stone at the foot of the grave or where ever you please.
