The "Introduction" page is followed by"Appendix B (starting Pvt. Edward Curtis) followed by "Table of Contents" followed by Appendix A "Table of Organization" then pages 2 - 20 and "Bibliography". Then I have "Table of contents" again, followed by Appendix A, Table of Organization again, followed by Introduction (but not the second page of Organization) followed by Appendix B beginning "Pvt. Edward Curtis (again), followed by Appendix C.
That is, two of the 11x17 pages were not only mis-assembled, but also misprinted. Hope that this helps.
