Also, a funeral director or graveyard management usually maintain the forms necessary and may help you through the process.
I was fortunate in that the gravard in Reno, NV where a Confederate veteran lay buried without stone since 1920. The graveyard director took my military history file, filled out the paper work, ordered the head stone, set it, and didn't charge a bit.
