The copy I had has been sent to another researcher. It was of only fair quality and not suitable to make a copy from that would have been readable. The citation for the microfilm of the pension application is as follows:

The microfilm can be ordered from two sources:
1. Interlibrary Loan. Will cost $1.00 for a one month loan but take about 2 months to receive.
2. LDS. (Film #1001533). Will cost $5.50 for a one month loan and usually takes about a month to receive.
You could also try just ordering a copy of the application through Interlibrary Loan. But, I am not sure how good the copy might be.
