Sorry not to have replied earlier. Burwell B. Lewis was 2nd Lieut of Co "B", 2nd Alabama Cavalry, serving under Capt John P West. Thomas Hall Lewis, formerly an officer of the 5th Alabama Regt, received a commission dated May 30, 1862, as Captain of an independent company of cavalry from Dallas County. This company later operated from Talladega after the Camp of Instruction went into operation. He must have been the third captain that you mentioned,

The great puzzle is which quartermaster depot supplied the three companies that you mentioned. In the summer of 1862 there probably wasn't anything nearer than Tuscaloosa or Jacksonville.
