One of the things I have found that made a difference is the medical practise of the Confederates. They tended as soon as possible to farm out their sick to individual houses and gave more personnal care to the sick than was the practise in the northern hospitals, where the sick tended to be massed together and attended by fewer stewards.

While they may have not known about "germs" per se they did know about infections (septis) and Sanitation conditions. The southern Doctors not have access to the most up to date medicines relied on Homeopatic remedies which tended to be mostly (sometimes powerful) laxitives and diuretics.
