Can anyone tell me the name of the document or card an enlistee has recorded that lists his parents or next of kin. I saw it online researching relative in the 10th Michigan Infantry. I got so excited I forgot to bookmark the page and the copy I tried to make didn't turn out. Can't remember website, had been searching for hours when I saw it. I sent off for his Civil War packet but that document was not included. Thanx
