In Ezra Warner's great "Generals in Gray" he states Gladden was wounded on the first day by a fragment from a shell and that his arm was amputated on the field. There was probably an ugent need to immediately remove the arm. I'm betting the surgery was performed on April 6, 1862.

I'll keep looking to see if we can find anything else.
