5th Ky Inf CSA

I am very curious about the military background of my 3rd great grandfather James L. Jervis/Jarvis. I visited his grave site and he has a new military headstone which reads 5TH SERG JAMES L JERVIS 13 KY CAV CO E 5 KY INF CSA. I have tried to find him in all the rosters but have only found James Jarvis, union, infantry ,40th Ky Inf. Co. E. He lived in Floyd Co.,Ky and was said to have came to Floyd county about 1846 to train soldiers ?. Could someone point me in the right direction so I can learn more about his service. Do you have to have some kind of proof or record of service to recieve the military headstone? Any help would be greatly appreciated, Patty
