Civil War Medical Care

My gggrandfather of the 25th Ark was wounded at the battle of Chickamauga and treated at Lee Hospital in Lauderdale Miss. According to his pension papers he had "4 in. sq. shot out of top of head, left eye out, left ear deprived of hearing". My question is considering the primitive medicine of the day, how would this wound have been treated? Would the doctors have placed some type of plate in his head to cover the wound? The treatment was successful as he lived until 82 years of age.
