The reference "killed after surrendering" was in Dr. Michael R. Bradley's "Nathan Bedford Forrest's Escort and Staff". I am at the office and my copy of the book is at home. I will send you the page reference and exact quote later this evening.

In the meantime, I believe I saw it in a roster found at the back of the book listing what happened to various members of the Escort.
