Lewis' Regiment may be Lewis' Battalion, Armistead's Brigade. It was led by Major Thomas Hall Lewis. Here are my notes on the organization of that command:

On April 26, 1864, General Leonidas Polk ordered ten companies on duty with the Conscript Bureau to assemble at Montgomery, Alabama. These were expected to form a cavalry regiment under command of Bryan M. Thomas of General Withers’ staff. However, companies commanded by Captains Chisholm, Love, McDonald, Moses and Smith failed to report, and shortly after May 18, 1864, organization of Thomas’ Alabama Cavalry Regiment was abandoned. Captain Lewis was appointed Major and assigned to command the remaining five companies. Also called Harrell’s Battalion after the death of Major Lewis, officers and men mustered at Blue Mountain, Alabama, June 10, 1864.

The name Hewlett makes the other unit easy to identify. William A. Hewlett was Lt.Col. of the 13th Alabama Partisan Ranger Battalion, which was merged with Boyles' Battalion to form the 56th Alabama Partisan Ranger Regiment. Here's the company information:

Company “F” of Walker and Jefferson Counties

Capt. Elijah J. Rice. Formerly Captain of Company “B”, 13th Alabama Partisan Ranger Battalion.

Accepted in Confederate service at Jasper, Alabama, September 6, 1862, officers and men had formed Company “B”, 13th Alabama Partisan Ranger Battalion. This company became Company "F" of the 56th Alabama on June 8, 1863.
