In the Army of the Trans-Mississippi Department, the head surgeon was called Medical Director and had the rank of colonel. I assume that the other field armies had a similar position (any assumptions concerning the Confederate army being very dicey at best). Some time ago I ran across a reference to a medical officer who held the rank of brigadier-general, but I can't recall if he was the head of the medical bureau in the War Department, or perhaps a Medical Director of one of the larger armies (ANV or AT).
