Hayes, This could be the case that there was a third flag. In writtings that my great-grandfather of the 40TH wrote, he states that new flags were issued by the government sometime after the Battle of Lookout Mountain and before the Atlanta Campaign. Also during this time period a new flag was issued to the 38TH Ala. Inf. that is now in the ADAH that is identical in style to this of the 40TH (not with sewn on battle honors). It is very unclear about what happened at Vicksburg to the 40THs flag. Oh yes, by the way this T.W. Coleman that you mention was a Captain of Co. F of the 40TH.
